As a remind the report/information should answer five questions for the hospital receiving the patient.

1. What type of room needed for this patient (triage, trauma, general, etc.)
2. What types of medications needed (continuation of meds given, drips)
3. What type of equipment is needed (ventilator, ECMO, dialysis, etc.)
4. Who needs to be in the room to help (respiratory, cardiologist, trauma, etc.)
5. Arrival time
